Paper Forms for Youth Group

Completed paper forms can be emailed to Lucy Tamajon ([email protected]) or should be returned to the Parish Office with Baptismal documentation for those not baptized at Our Lady of the Angels.

Parish Office: MON. - Noon - 4pm, TUES-THUR. 8:30AM - 4:00 PM or FRI. 8:30 AM-12:00 NOON

There is no fee for joining youth group.
